§ 46-23. Application.

  • Application for a certificate of registration shall be made upon a form provided by the Chief of Police. The applicant shall truthfully state, in full, under oath, the following information:

    (1)

    Applicant's name; residences during the past three years, including the time the applicant lived at each residence, business address if different than current residence address and Social Security Number;

    (2)

    A physical description of the applicant or a copy of the photograph on the applicant's State-issued driver's license, or identification card;

    (3)

    The name and address of the applicant's employer or person the applicant represents and the length of time of such employment or representation;

    (4)

    A description of the subject matter of the solicitation;

    (5)

    A proposed term for the certificate of registration, which shall not exceed one year;

    (6)

    The date, or approximate date, of the most recent application for a certificate under this Article submitted by the applicant, if any;

    (7)

    Whether a certificate of registration issued to the applicant under this Article or any similar certification issued by any other municipality has ever been revoked;

    (8)

    Whether the applicant has ever been convicted of violating any provision of this Article;

    (9)

    Whether the applicant has ever been convicted of a felony under the laws of the State, any other state or the United States.

    Each applicant shall submit to fingerprinting and background investigation as part of the application process. The applicant shall pay the sum of $65.00 at the time of application to cover the Village's burdens and expenses related to the fingerprinting and background investigation.
